What is on the page

The period you ask for

Thirty days, three or six months, and what the person wants to bring forward, such as movement changes or medicine timing.

Symptoms and changes noted

What was noted, and how many times in the period.

Current medicines

Dose, times of day and start date.

Checks, readings, questions

Readings and checks, and the questions the person kept. It says what the record doesn’t hold for the period.

For people who see more than one specialist.

Michel has Parkinson’s and type 2 diabetes: four medicines from three prescribers. His summary can be ordered for the specialist being seen, a neurologist or a diabetologist, from one record. That is multimorbidity in its plainest form: one person, several clinicians, one set of dated entries.

What it does not do

  • It doesn’t diagnose, score or sound alarms.
  • It doesn’t send anything to a clinic. Nothing is exported or sent when a summary is prepared.
  • There is no clinician login and no feed to watch. The person brings the page.
